What is AI Form for Google Docs?

AI Form for Google Docs is an iPhone and iPad app for creating and managing Google Forms without a computer. It is made by FormsGen.

Create with AI: describe the form you need in one sentence. Choose the form type (survey, quiz, registration, feedback), the number of questions, the tone and the language, and review the questions before the form is created.

Scan a paper questionnaire: on-device text recognition turns a printed form into an editable one. Questions, answer options and 1 to 5 rating scales are detected. The photo does not leave the phone.

Native editor: multiple choice, checkboxes, dropdown, short and long answers, linear scale, rating, date, time, grids, sections, images and descriptions on questions and options, drag to reorder.

Quizzes: set the correct answer and points for each question. Google Forms grades every submission.

Share: the respondent link, a fullscreen QR code (save as PNG or SVG), a printable PDF, or an image for social media.

Responses: a chart for each question, each person's answers, and export to CSV, Excel (XLSX) or Google Sheets.

Privacy: forms and responses stay in the user's own Google Drive; the app does not store them on its servers.

Languages: English, French, Arabic, Portuguese (Brazil), Spanish (Mexico) and Japanese.

Pricing: free to download. Creating, editing, scanning, charts and exports are free with ads. Premium (weekly, yearly or lifetime, with a 3-day free trial) unlocks the AI builder, 16 of the 17 templates, and the QR code, social image and PDF after 3 free uses each, and removes ads.
