What is FoxForm?

FoxForm is a form, quiz and survey builder for forms that qualify rather than only collect.

Each answer option can carry points, which accumulate into a score. That score decides which screen comes next and which result page appears at the end, so a form can finish with a quote, a diagnosis or a rating band instead of a confirmation message. Calculations run inside the page through calc() expressions over answers and variables, which covers pricing estimates and ROI figures.

Conditional navigation uses AND/OR condition groups, and individual questions can be shown or hidden based on earlier answers. There are 17 question types, including short and long text, number, email, phone, dropdown, checkboxes, multiple choice, rating, opinion scale, NPS, slider, date, file upload and picture choice.

Forms can be generated from a written prompt. A REST API and a hosted Model Context Protocol server expose the same operations to AI agents: creating a form, publishing it, reading responses and retrieving per-screen analytics.

Published forms are rendered as static HTML and served from a CDN. Built-in analytics show views, starts, completions and drop-off per screen, and every response can fire a webhook.

Accounts are created without a card and start on a free plan. The first 25 responses to a form are free with no time limit, and the form stays live and keeps collecting after that; a paid plan is required to read the responses beyond the first 25.

Workspaces support team collaboration with Owner, Admin and Member roles. Available in English, Portuguese and Spanish.
