Form Tools is a powerful, flexible, free and open source PHP/MySQL script you install on your website for managing your forms and data You can use Form Tools to house the data from your own forms, create private forms for use by specific users only, or create forms right on your website - all with no programming knowledge needed. Form Tools offers a range of extra functionality through optional modules, letting you tailor your installation for your own project requirements. And if the module doesn't already exist, you can always write your own! Create as many accounts as you need. Accounts can be set up to show exactly which forms, and form fields you want the user to see. Control read and write privileges, what data export options are available, and much more. Form Tools is much more than just a data gatherer. It lets you visualize your form submission data through the Data Visualization module and provides a simple way to export your data for use in whatever other applications you need. With over 50 translations available. An API is available for people wishing to do a deep integration with Form Tools. The API provides ways to submit form data via code rather than form POSTs, helper functions for things like maintaining sessions, displaying captchas, and much more.

This is exactly what I needed, however, the forums are dead, there is no responsive support whatsoever, and the script has lagged so far behind that it won't install on my website because it requires MySQL 4 and if you have anything beyond that the script doesn't install. (I have MySQL 5.
Form Tools definitely has some good functionality, but it was a little over my head in terms of usability. Also, I wanted something that could integrate into my WordPress site easier, preferably in the form of a plugin. After some searching, I actually found a plugin that met my form-building criteria, along with a completely separate need I had (client portals). The WP-Client plugin gave me the client portal functionality I was looking for, and their recently released Form Builder add-on was exactly what I needed in terms of creating custom forms to display for my clients. Between the two, it was exactly what I was looking for, and I was happy to be able to find something that so seamlessly integrated with my WordPress site. I would definitely recommend giving WP-Client and their Form Builder add-on a look if you are in the market for something similar.
