Upload a PDF, a Word or Excel file, a scan, or a photo of a printed page. The questions are read straight off the document, keeping the wording you already use, so there is nothing to build and no form editor to learn. You get a shareable link that works on any phone.

The person filling it in never has to create an account. Answers come back as a spreadsheet, or as your original PDF with the fields filled in, so the file you hand on looks like the one you started with.

It reads scans and phone photographs, not only digital PDFs, which matters if the form you need to digitise only exists on paper.

Free for one form of up to five pages and 100 responses a month. Paid plans add more forms, higher response limits, larger documents, extra seats, removal of our badge, and custom domains.
