Fixed, written rules check that the totals add up and point out charges or codes listed more than once, quantities above the unit limits Medicare publishes, and routine supplies billed on their own line. A few optional questions look for money you may get back or may not owe: a payment above your share, a share above your plan's yearly out-of-pocket maximum, or a possible surprise bill. Each item comes with the reason, the question to ask and a letter for the billing office.

Asking often helps: in a 2024 Commonwealth Fund survey, 38% of insured adults under 65 who disputed a medical bill had it reduced or eliminated.
