The setup wizard says "It will not change or modify your precious files, it only indexes them".
That is not a feature. It means KPhotoAlbum can't store metadata (e.g. EXIF tags, ratings) in the image files, so other tools won't be able to benefit from the organization you've done with KPhotoAlbum.
