I've used quite a few form builder plugins, and I think GravityForms is definitely one of the best. I use it in conjunction with another plugin called WP-Client to provide custom private forms for my site members to fill out. I can easily create the desired forms using GravityForms, and then insert shortcodes from the plugin into private pages that I create using WP-Client. My site members can then login via the login page on my site (also setup using WP-Client), and easily view and fill out the forms I have assigned to them. It's a great system and I absolutely love it!
