The best Self-Hosted alternative is Isso, which is both free and Open Source.     Cusdis is one of the easiest solutions to implement and customize that I've tried. There's a free (limited) hosted version and you can easily self-host. Only downside is that it lacks the fancier features of Disqus and a spam filter.
