Easy to use, free online with a Office.com account (no need to have a Office 365 subscription). Simple question branching/logic. Excel data export. A good alternative to Google Forms.

If you already have a subscription to Office 365, it's a handy little web app for basic quizzes and surveys. Not well advertised though, I don't think many know it exists.

I had high hopes for this one...

It is now available to everyone. In 2017 you can only try it with an education or work subscription to Office 365 via:

Edit April 2020: It seems no significant features have been added.
Microsoft Forms is easy to use, but I do not see any advantage over Google Forms.

Please be aware of it's limits - users can't go back to finish answers, there is a 100 question limit, choice fields only store 60 options, max 4000 characters in user responses and the likeart scale is limited to 20 questions...
